Breakthrough - Transitional Housing

Breakthrough’s Transitional Housing Program works alongside men and women to secure permanent or more stable housing and increase income within a 120-day goal. This is a housing-first, trauma-informed program that provides a dedicated bed, care coordination, and housing retention support.

Breaktrhough - Permanent Housing

Breakthrough’s Permanent Supportive Housing program provides stable housing for individuals and families. As a participant, you will receive case management support to identify, set, and reach goals related to stability, well-being, and growth. Breakthrough manages 43 single apartments across Chicago and 11 units of housing for families.
